Head: allergies, sinus infections, conjunctivitis (pink eye), outer or middle ear infections
Throat: strep throat, sore throat
Chest: bronchitis, cough (may incur additional treatment costs)
GI: heartburn,
Urinary Tract: bladder infections
Musculoskeletal: sprains, strains. (Additional charges for any splints, etc)
Skin: allergic rashes (poison ivy), athlete's foot, cold sores and impetigo, tick bites (may incur additional lab costs), minor burns, scrapes, minor rashes, minor skin infections, minor cuts (may incur additional treatment fees), scalp conditions, minor sunburn, ringworm, warts

All above conditions $49.  Additional charges may
apply if other testing or treatment required.
